Registered Nurse (RN) - Care Plan Coordinator If you are passionate about coordinating patient care plans, join us as a Registered Nurse (RN) - Care Plan Coordinator. In this role, you will collaborate with interdisciplinary teams to develop individualized care plans that optimize patient outcomes while ensuring seamless communication across departments. Key Responsibilities:

• Assess patients' needs upon admission and develop comprehensive care plans tailored to their medical conditions and recovery goals.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to implement effective care plans.
• Monitor patient progress regularly and adjust care plans as needed based on clinical outcomes or changes in condition.
• Educate patients and families on treatment plans, discharge instructions, and available resources for continued care at home or in community settings.
• Document care plan details accurately in patient records while ensuring compliance with regulatory standards. Work Environment:
